You may give it to your coach, drop it of at the office through mail slot in the door MSYSA Heads Up Concussion Information for Parents and players Form Click here for the Return To Play Form for players More information about the Michigan Public Acts 342 and 343 There is a new State mandated computer training video that all Coaches have to watch and take a short test. Takes about 30 minutes. Make sure you use the name on your license for this and Risk Management information. Coaches you will receive a certificate for completing and you only need to watch the video once in your coaching career. |
