Game Reschedule Form

	Information required when requesting to reschedule a game

· Only the HOME team needs to fill out this request.

· YOU MUST CONTACT THE OPPOSING COACH AND AGREE ON YOUR NEW GAME TIMES. Confirming from the Master Schedule that the field is currently available

· We will try our best, but do NOT guarantee that your request will be honored.

· When the game is rescheduled, you and the visiting coach will receive a confirming E-mail

· Please make sure all the information you enter is accurate.

· Other than a referee canceled game, you MUST SUBMIT YOUR REQUEST BY

Fall Season: AUGUST 25.

Spring Season APRIL 30.

Other than referee canceled games, no games will be rescheduled after April 30 or September 16.
